Request to Change Music Lessons

Please use this form to request a change to your regular ongoing music lessons. Changes include day(s) and time(s) of lesson, location of lesson, music instrument(s) or focus of lessons, and teacher.

A one-time administrative fee of $25 per student will be charged for each change request fulfilled.

Please remember to submit your change requests by the 15th of the month prior to the requested change. (Mid-month change requests will be honored but may require several weeks’ lead time.)
